HERE'S HOW TO QUALIFY

• You must be 18 years of age or older.
• You must have full-time verifiable employment
• Your monthly income must be over $1,000 take home
• You must have direct deposit into you checking account
• You must have a valid state driver's license or state issued Id
• You must have a checking account in good standing and preprinted checks
• You must have a home phone in working order
• You must be paid Weekly, BI-Weekly or Semi-Monthly
• You must have access to a fax machine so you can send any and all requested documents
• You must be able to provide a current bank statement no older than 30 days

You'll need to print out*, read, complete and sign the enclosed application and fax the following documents to us:

Completed Application
Copy of a Voided Check
Complete copy of your most recent bank statement
Copy of your current pay stub.
Copy of a valid Driver's License or State ID
